Data Verification & Standard Release Forms

Parents and/or students are asked to come to the school to pick up the Data Verification Form and Standard Release Form  from August 27^th to August 30^th, between the hours of 7:30 a.m. and 3:00 p.m.  Both forms need to be reviewed and signed by a parent (or student, if over 18). However, signed DVFs and Standard Release Forms may be handed in to the main office from August 27^th to August 30^th in exchange for a timetable. DVFs that are not picked up prior to the beginning of school will be sent home on the first day of school for parent review and a signature.  *Students 18 and over who are unable to pick up their DVF must provide a note permitting a parent/guardian/sibling to pick it up.
